This potion has lost its once beneficial magical abilities and has become a potent poison. The imbiber must make a DC 16 Fortitude save or take 1d10 points of Constitution damage. A minute later he must save again (DC 16) or take 1d10 points of Constitution damage.

Prerequisites: Craft Wondrous Item, poison

Cost to Create: 2,500 gp, 200 XP, 5 days.

Source: Dungeon Master’s Guide (version 3.5), page 276. This item is messed up in subtle ways in the Caster Level and Aura entries. No actual potion should have a caster level exceeding 5th, since a potion can only use up to 3rd level spells, so the aura would be a dead giveaway that something is amiss. Still, it could be taken for an elixir, which can be more powerful. (The prerequisite for actually making this item deliberately suggests that as well.) The next problem is that a CL of 12 would indicate a strong aura, not a moderate one. It was decided to drop the caster level to 11th to bring it in line with the aura.
